Please read the following information concerning FSA's legal status, state-required enrollment documents, state-allowed Religious Exemptions, HEP Termination, and Academic Recordkeeping. Then continue to the Enrollment Form via the link at the bottom of this page.
LEGAL STATUS: Freedomschoolers Academy is a Private School for home educators. We are a legal entity operating under Florida's private school laws. Our families do NOT submit a letter of intent to the county Home Education Program (HEP). The county HEP is for those homeschooling under Florida's homeschool laws.
All state-required documents must be submitted with your enrollment either by uploading with the Enrollment Form or by email to [email protected]. Religious exemption* statements can be elected on the enrollment form.
ENROLLMENT DOCUMENTS: The following documents are required for private school enrollment per FL Statutes 1003.22, 1002.42:
* RELIGIOUS EXEMPTIONS: The Religious Exemptions for Health Exam and/or Immunization Records under Florida Statute 1003.22 are as follows:
- A photocopy of the student's birth certificate (DO NOT send originals)
- School Entry Health Exam form Florida DH 3040, performed within 1 year prior to enrollment OR election of religious exemption statement*.
- Immunization Record form Florida DH 680, current information no more than 1 year prior to enrollment OR election of religious exemption statement*.
* RELIGIOUS EXEMPTIONS: The Religious Exemptions for Health Exam and/or Immunization Records under Florida Statute 1003.22 are as follows:
- Florida Statute 1003.22(1) provides in relevant part that "a child shall be exempted from the requirement of a health examination upon written request of the parent of the child stating objections to the examination on religious grounds".
- Florida Statute 1003.22(5)(a) provides that "[t]he provisions of this section shall not apply if...[t]he parent of the child objects in writing that the administration of immunizing agents conflicts with his or her religious tenets or practices".
Former HEP Families: If your student(s) is/are being withdrawn from a Home Education Program (HEP) in a Florida County, you may use the HEP Termination Form. Return the completed and signed form to [email protected] and it will be forwarded to the former HEP with our Enrollment Verification email. You will be BCC'd on that email.
