Freedomschoolers Academy is a private school for Florida home educators seeking an alternative to the county Home Education Program (HEP). Whether you’re currently registered under HEP, transferring from a public or private school, or relocating to Florida — this page will guide you step-by-step.
All HEP, Public or Private School, or Relocating Students
  • Complete a Records Release/HEP Termination form to be submitted with the Enrollment Form.
  • Failing to submit this form may cause delays and prevent a smooth transition from the former school or program.
  • Read the information below about state-required enrollment documents and Religious Exemptions.
  • Birth certificates are never optional or exempted.
  • Some individuals may need to attach documents to support custody or legal restrictions.
  • Attach all necessary documents with the Enrollment Form.

Students Never Enrolled in any HEP, Public or Private School.
  • If your student has never been enrolled in any school or registered with a homeschooling program, do not complete a Records Release/HEP Termination form.
  • Read the information below about state-required enrollment documents and Religious Exemptions.

State Required Enrollment Documents
Religious Exemptions
 Required documents for private school enrollment per Florida Statutes 1003.22 and 1002.42: 
​
  1. ​A photocopy of the student's birth certificate (DO NOT send originals)
  2. School Entry Health Exam form Florida DH 3040
    1. ​ performed within 1 year prior to enrollment OR 
    2. election of religious exemption statement.
  3. Immunization Record form Florida DH 680
    1. ​current information no more than 1 year prior to enrollment OR 
    2. election of religious exemption statement.

Note: Students relocating from outside Florida may submit similar Health Exam and Immunization Record forms from their previous state so long as it is current within 1 year prior to enrollment.
 The Religious Exemptions for Health Exam and/or Immunization Records under Florida Statute 1003.22 are as follows:
  • Florida Statute 1003.22(1) provides in relevant part that "a child shall be exempted from the requirement of a health examination upon written request of the parent of the child stating objections to the examination on religious grounds".
  • Florida Statute 1003.22(5)(a) provides that "[t]he provisions of this section shall not apply if...[t]he parent of the child objects in writing that the administration of immunizing agents conflicts with his or her religious tenets or practices".

To elect a Religious Exemption, please check the applicable box in the Parental Statement of Religious Exemption section of the Enrollment Form. 

There is no state requirement to have a physician or Department of Health official approve of any religious exemption statement or use any standard form. ​
